The report made five recommendations for improving hospital resources, recordkeeping, and protection of staff during medical investigations.
Prasitha called on the government to act on the recommendations.
“As a mother, all mothers should value their children’s achievements and share in their happiness, but I have my daughter’s death certificate and autopsy report.”

That night, the staff level was at the ratio set by the hospital, but Linton found the level to be insufficient.
Since February last year, Aswath has launched a petition asking the McGowan government to convene a Royal Commission on WA’s health system in October.
He repeated these calls on Monday.
“This is one of the reasons I have filed a petition for the Royal Commission, because we don’t want another case.
Aswath also suggested that WA Health executive director David Russell-Weisz should resign and a “new captain” be elected.
